Access & Copies: You may inspect and copy your medical records in writing. A reasonable copying fee may apply.
Restriction Requests: You may submit written requests to limit how we use or disclose your information. We are not always required to agree to such requests.
Confidential Communications: You may request that we contact you through alternative means, such as a work phone number or a different mailing address.
Amendment: You may request corrections to inaccurate or incomplete information in your records.
Disclosure Accounting: You may request a record of disclosures we have made outside of treatment, payment, and operations for up to six years.
Paper Copies: You have the right to receive a printed copy of this notice upon request.
Svelta Dental Design reserves the right to modify this privacy notice. Changes apply to information we already hold as well as information received in the future. The current version will always be posted on our website with the effective date clearly marked.
To file a complaint, contact our Office Manager at the address below or file directly with the Department of Health and Human Services. We will not retaliate against you for filing a complaint.
